What it is
An offline calculator that uses the AHA PREVENT equations. The calculation does not use the internet; the equations, coefficients and ZIP-to-SDI table are all inside this one file.

Privacy
Everything is computed on this device. Nothing is saved, and this page sends nothing. Anything you report leaves only if you paste it into the Google Form and press Submit there.
Closing or reloading the page, or Clear all data, clears it.
How it reads values
Paste a note: Epic PREVENT SmartPhrase or ASCVD SmartText, colleagues' SmartPhrases, typed notes, or other EHRs' lab tables.
Values read from the text go into the form for you to check (amber = please confirm, red = required and not found). Only the values in the form are used.
Accepted ranges (as on the official calculator): age 30–79 years, total cholesterol 130–320 mg/dL, HDL 20–100 mg/dL, SBP 90–200 mmHg, eGFR 15–140 mL/min/1.73m², BMI 18.5–39.9 kg/m². Values outside a range give no estimate (they are not clamped). The 30-year estimate is for ages 30–59 only.
Keeping it current
It does not update itself. Periodically calculate a patient on the official AHA PREVENT calculator (professional.heart.org) and compare the results.
Formula last verified against the AHA calculator: 2026-10-19 (8 test cases + 25 ZIP codes).
Last AHA update: October 14, 2026 (HbA1c correction).
